How it works
Three steps from referral to supports in place.
Send the referral
Use the referral form on our contact page. Include diagnosis, funding and any discharge or housing needs so we can move straight to assessment.
We assess and respond
We review capacity, funding fit and worker availability, then come back to you with a clear yes and a plan, or an honest no so you are not left waiting.
Supports commence
We match workers, confirm the roster and keep you updated as the coordinator. You stay in the loop from first meeting to steady state.
