What it is

Some providers delegate nursing tasks to support workers without clinical oversight. We do not. Nursing visits at Resilient Life Care are carried out by qualified nurses working to a documented care plan, with clinical notes after every visit and a clear escalation pathway if something changes.

Our nurses also work alongside our complex care and SIL teams, so clinical and daily support operate as one team, not in silos.

What's included

Clinical nursing, delivered at home.

Wound care & complex dressings

Assessment and dressing of acute and chronic wounds, with a documented wound management plan shared with your treating team.

Medication administration & review

Administration of prescribed medications including injections, with accurate documentation and liaison with prescribers.

Continence & catheter care

Continence assessment and management plans, plus catheter care including insertion, removal and monitoring.

PEG & enteral feeding

PEG site care, feed administration and troubleshooting, following your treating team's feeding regimen.

Complex bowel care

Nurse-led bowel programs for participants with spinal cord injury, neurological conditions or post-surgical needs, including stoma care.

Clinical oversight of support teams

Training, competency checks and escalation support for the workers delivering your high intensity supports.

Is this covered by the NDIS?
Community nursing supports are funded through NDIS plans where they relate to your disability support needs. We work with your support coordinator or plan manager to keep the funding side straightforward.
Do the same nurses come each time?
We keep your nursing team as consistent as possible, for the same reason we keep support workers consistent: familiar faces notice changes earlier and care better.
Can nursing care start straight after a hospital stay?
Yes. Through our Hospital Transitions service, nursing is in place before discharge day so clinical care continues without a gap.
