Getting started
How do we get started?
Three steps. First, a free, no obligation chat, no forms first, where we listen to your situation and understand your needs and the people involved. Second, we build a tailored support plan to match your needs and help you live as independently as possible. Third, we match you with the right support workers and your supports begin.
What funding types do you work with?
We support NDIS participants however their plan is managed. Tell us about your plan when you get in touch and we will explain exactly how the funding side works for your situation, in plain language.
Which areas do you cover?
We support participants across Victoria, New South Wales, Queensland and Tasmania, with offices in Melbourne (Preston) and Brisbane (Chermside), both open by appointment. Not in one of these areas? Get in touch anyway. We're happy to help and find a solution that works for you.
Who can make a referral?
Anyone. Participants and families can contact us directly by phone, email or our online form. Support coordinators, hospital social workers and NDIS liaisons can use the secure referral form on our contact page and our intake team will respond with next steps.
Hospital transitions
Can you help me leave hospital?
Yes, hospital transitions are one of the things we do best. We work with you, find you accommodation where required, and build a support team that fits your needs so you are supported straight out of hospital. We also focus on setting you up for success: matched with the right support workers and receiving tailored care from day one at home.
How quickly can support start?
Every situation is different, so we don't promise a set time frame. What we can promise is that we work quickly, we treat urgent discharges with urgency, and we keep you informed at every step so you always know where things stand.
Housing
Do you provide housing?
We can connect participants to a range of housing options across Victoria and Queensland including Specialist Disability Accommodation, Medium Term Accommodation, Short Term Accommodation and Supported Independent Living. See our Housing pages for current homes and vacancies.
Workers & quality
Are you an NDIS registered provider?
Yes. Resilient Life Care is an NDIS registered provider, and we have an incredible team of dedicated support workers behind that registration. Being registered means independent auditing, screened workers and national quality standards on every shift.
Do you offer aged care supports?
Yes. We are a registered provider with My Aged Care. Disability support is our core focus, and if you or a family member are exploring aged care options, contact us and we will point you in the right direction.
How do you choose my support workers?
We don't roster strangers. We introduce you to a support worker chosen for personality, languages, lifestyle, personal preferences, and the actual care you need. If a match isn't right, tell us and we will fix it.
Do you provide nursing care?
Yes. Our nursing care includes community nursing and high-intensity daily personal activities, delivered at home and coordinated with your everyday supports so nothing falls between the gaps.
What checks and training do your support workers have?
Every support worker holds current NDIS Worker Screening and Working with Children checks, along with relevant qualifications in Disability Support, Individual Support (Ageing) and other industry-recognised certifications. Ongoing professional development keeps the whole team current.
How do you keep families informed?
We keep communication clear, proactive and jargon-free, so families stay informed without having to chase updates. Less time managing support, more time enjoying the moments that matter.
