Hospital transition process

Also known as our Rapid Hospital Discharge Program.

  1. 01First meeting. In hospital or by phone. No forms first, just a conversation about the participant and what they need.
  2. 02Hospital liaison. We work directly with the hospital team and the NDIA so nothing is lost in handover.
  3. 03Match The Right Support Workers. Workers chosen for personality, language, culture and the care actually needed.
  4. 04Support planning. Planning meetings with the participant, family and hospital to map the supports required at home.
  5. 05Housing, where required. SIL, SDA, Medium Term or Short Term Accommodation lined up before discharge.
  6. 06Discharge day. The home is set up and the support team is at the door, not on a waiting list.
  7. 07Settle in and check-ins. Support continues at home, with regular check-ins and adjustments as needs change.
