Hospital transition process
Also known as our Rapid Hospital Discharge Program.
- 01First meeting. In hospital or by phone. No forms first, just a conversation about the participant and what they need.
- 02Hospital liaison. We work directly with the hospital team and the NDIA so nothing is lost in handover.
- 03Match The Right Support Workers. Workers chosen for personality, language, culture and the care actually needed.
- 04Support planning. Planning meetings with the participant, family and hospital to map the supports required at home.
- 05Housing, where required. SIL, SDA, Medium Term or Short Term Accommodation lined up before discharge.
- 06Discharge day. The home is set up and the support team is at the door, not on a waiting list.
- 07Settle in and check-ins. Support continues at home, with regular check-ins and adjustments as needs change.
